Fees for Service
• If you have insurance and we are in-network, you would be responsible for your typical medical copayment.
• If you are uninsured or out-of-network, we provide a sliding scale fee with proof of financial documents.
• Emergency call sessions requires payment of regular copay.
• Requests for forms and letters are granted within reasonable time frame for completion.
Sliding Fee Scale Program
Embrace Therapy is devoted to providing mental health treatment to all who are in need. Our dedication is shown in having several insurances paneled with the practice to facilitate access to mental health treatment.
In addition, we have enrolled a sliding fee discount policy program for those who have financial limitations which prevent them from receiving clinical services.
• A sliding fee discount is available for eligible patients.
• No one will be denied access to services due to an inability to pay.
• A discounted/sliding fee schedule is available based on family size and income.
• Federal Poverty Guidelines are used to determine eligibility.
• If you or your family have a household income at or below 200% of the Federal Poverty Guidelines, you may qualify for reduced fees.
• No patient will be denied services because of inability to pay.
